Berkeley EDGE MENTORSHIP PROGRAM

MENTEE AGREEMENT 

The Berkeley EDGE Mentorship Program partners students with UC Berkeley alums.  The goal of the program is twofold - to assist students in achieving success during their time at Berkeley, and to create an opportunity for alumni to give back to the student community. 

Both parties (mentor & mentee) are required to follow the program guidelines to ensure the integrity and success of the program. 

  1. I agree that I will make initial contact with the Mentor and that he/she will not be given any information other than my name, major and interests.
  2. I agree not to enter into a consensual, romantic relationship with the mentor while participating in the Berkeley EDGE mentorship program.
  3. I agree there will be no physical contact of a romantic nature with the mentor while participating in the Berkeley EDGE mentorship program.
  4. I agree to respect the UC Berkeley University Codes of Conduct.
  5. I understand that at any time during my participation in the program I can contact the California Alumni Association with any concerns, problems or issues.
  6. I agree that I or the mentor may terminate the relationship at any time.
  7. I agree not to share the mentor's contact information without permission.
  8. I agree that any information shared by the mentor is considered confidential unless stated otherwise. 
  9. I agree that my information may be shared with representatives from CAA and or other official University of California representatives.
  10. I agree that the California Alumni Association (CAA) may terminate a mentor/ mentee relationship at any time.
  11. I understand that all mentors participating in the mentor program will be required to agree to a similar agreement.
  12. I agree that at any time I need to remove myself from the program, I will inform the CAA representatives as soon as possible and that I can request that CAA inform the mentor.

  1. I agree that I understand it is the Mentee's prerogative to select a Mentor and that he/she will be given access to information that I create in my profile once I am selected as a potential match.
  2. I agree that I can make initial contact with any other Mentors and that they will be given access to information that I create in my profile.
  3. I agree not to enter into a consensual, romantic relationship with the mentee while participating in the Berkeley EDGE mentorship program.
  4. I agree there will be no physical contact of a romantic nature with the mentee while participating in the Berkeley EDGE mentorship program.
  5. I agree to respect the UC Berkeley University Codes of Conduct.
  6. I understand that at any time during my participation in the program I can contact the California Alumni Association with any concerns, problems or issues.

  9. I agree that any information shared by the mentee is considered confidential unless stated otherwise.
  10. I agree the only exception to item number 9 are in those dire cases when a mentee may share information with me that leads me to believe the mentee may be in danger to him/herself or others around them. In these cases, I agree to contact the CAA immediately to notify them of the situation.
  11. I agree that my information may be shared with representatives from CAA and or other official University of California representatives.
  12. I agree that the California Alumni Association (CAA) may terminate a mentor/ mentee relationship at any time.
  13. I understand that all mentees participating in the mentorship program will be required to agree to a similar agreement.
